Physicians: Register to become an FMM Participant

FMM Online Program and Discussion areas on this web site are secure and restricted to physicians only. You may want to register now if:
  1. You are already running an FMM group in your community;
  2. You are planning to start an FMM group and need the "starter kit" and other materials;
  3. You would like to be a part of an online FMM discussion group;
  4. You are medical school/residency faculty and would like to use these materials in student and resident teaching, and to network with other faculty who are adapting the FMM process in their programs.

There is no obligation to ISHI if you register; we do not expect everyone who registers to start an FMM group.

About the Registration

  • To access the private physician areas of this web site, you need to become a registered participant. When you send us your registration, we will email you within 3 business days to confirm your initial user name and password. Then, join us as often as you would like!
  • After you've registered, you can enter the secure FMM Discussion areas with a simple login to the site.

Security and Confidentiality

  • Your registration information will be kept strictly confidential. We do not give or sell the FMM participant list to anyone.
  • Community FMM group facilitators may choose to provide an email address, which will be posted in the secure physician-only area. This contact information will enable physicians in your community who want to join an existing, live FMM group to contact you.
  • The discussion areas on this web site are password protected and all discussions are confidential. By registering, you are agreeing to maintain this confidentiality.

Thank you. Continue registration...
